Overview
Five different problems under one name
People use “body contouring” for treatments that address stubborn fat, weak muscle tone, loose skin, cellulite, and surplus skin after weight loss. Those are five separate problems. No one treatment fixes all of them, and the FDA has cleared or approved each product for named uses only:
- Fat: CoolSculpting (FDA clearance K233732) and SculpSure (FDA clearance K182741) are cleared to break down fat in listed body areas.
- Abdominal fat and circumference: Emsculpt Neo carries clearance for both (FDA clearance K192224).
- Skin: Morpheus8 is cleared for procedures that need soft tissue coagulated or contracted (FDA clearance K231790).
- Cellulite: Avéli is cleared for the look of cellulite on adult women’s buttocks and thighs (FDA clearance K221336).
- Surplus skin: that is a job for surgery. ASPS describes a tummy tuck as removing excess skin and fat and, usually, repairing abdominal muscles that have separated or weakened (ASPS on tummy tuck).
The useful first sentence at a consultation is the one that names which of those five you care about. It changes the shortlist. Loose skin is a good example: ASPS says it, or poor tone, can make cryolipolysis a weak choice (ASPS on cryolipolysis).
Three recent developments are worth knowing before you book anywhere in the Dallas area:
- GLP-1 weight loss includes muscle. In a SURMOUNT-1 substudy, about 25% of the weight lost on tirzepatide was lean mass, the same proportion as on placebo (Look et al., 2025).
- Newer PAH estimates are higher. Recently published rates of paradoxical adipose hyperplasia after cryolipolysis sit above the first estimate (Mah et al., 2025).
- The FDA has issued safety communications about Renuvion/J-Plasma (FDA on Renuvion/J-Plasma) and about RF microneedling (FDA on RF microneedling).
| Quick facts | What the sources say |
|---|---|
| Dallas cost | This site lists no Dallas price range for body contouring |
| Time in the chair | Cryolipolysis takes about an hour (ASPS on cryolipolysis). One study of a 1060 nm laser used a single 25-minute session (Kislevitz et al., 2021) |
| Downtime | For cryolipolysis, ASPS lists no activity limits (ASPS on cryolipolysis). After Avéli, 73.5% of study patients were back to normal activities inside a week (FDA clearance K221336) |
| When results show | Cryolipolysis: treated fat cells are cleared over 4 to 6 months (ASPS on cryolipolysis). Avéli: benefit shown out to one year (FDA clearance K221336) |
Fat Devices
Reducing fat without surgery
Non-invasive fat devices use one of three approaches: cold, heat, or heat paired with muscle stimulation. Two limits apply to all of them. First, they are not weight-loss treatments: ASPS puts the average fat reduction in a cryolipolysis-treated area at about 20 percent (ASPS on cryolipolysis) and says no non-surgical option replaces liposuction (ASPS on nonsurgical fat reduction). Second, the body-area clearances for CoolSculpting (FDA clearance K233732) and SculpSure (FDA clearance K182741) apply only up to a BMI of 30.
Cold: CoolSculpting Elite
CoolSculpting Elite cools the skin over the fat toward a target temperature of as low as -11 °C (FDA clearance K233732), and the body removes the injured fat cells over the next four to six months (ASPS on cryolipolysis).
Its clearance lists cold-assisted lipolysis of the upper arm, bra fat, back fat, banana roll, thigh, flank and abdomen for people with a BMI of 30 or lower. For the submental and submandibular areas (beneath the chin and along the jawline), the BMI ceiling rises to 46.2 (FDA clearance K233732).
ASPS describes a session of about an hour and an average fat loss of about 20 percent, and lists asymmetry and surface irregularities among the risks. It also says cryolipolysis is not for people with cold urticaria, paroxysmal cold hemoglobinuria or cryoglobulinemia (ASPS on cryolipolysis). The section below covers paradoxical adipose hyperplasia, the complication most worth asking about.
Laser: SculpSure
SculpSure uses a 1060 nm laser. It is cleared for non-invasive lipolysis of the thighs, back, flanks and abdomen at a BMI of 30 or lower, and of the submental area at a BMI of 49 or lower (FDA clearance K182741). A single-arm study treated the abdomens of 29 patients once, for 25 minutes, with a 1060 nm diode laser; at 12 weeks, ultrasound measured an 8.55% fat reduction (Kislevitz et al., 2021).
Radiofrequency: truSculpt
truSculpt heats tissue with radiofrequency energy. Its FDA file reports that the applicator held the skin surface at 40 to 45 °C. The clearance covers non-invasive lipolysis and reduction of abdominal circumference, with the 2 MHz mode and the 40 cm² handpiece (FDA clearance K223110).
Radiofrequency plus muscle stimulation: Emsculpt Neo
Emsculpt Neo adds muscle stimulation to radiofrequency heating, and its FDA file states a therapy temperature of 40 to 43 °C (FDA clearance K192224). Its evidence is covered in the muscle section further down.
PAH
Paradoxical adipose hyperplasia after cryolipolysis
With paradoxical adipose hyperplasia (PAH), a treated area gets a firm, larger fat deposit instead of a smaller one. The published rates do not agree:
| Source | Rate reported |
|---|---|
| The first published report (Jalian et al., 2014) | 0.0051% |
| A single practice (Singh et al., 2015) | 0.47%, or 2 in 422 treatments; the authors called this 100 times the rate the manufacturer reported |
| 8 Canadian centers, 8,658 cycles (Nikolis and Enright, 2021) | 0.05% to 0.39%; rates dropped by more than 75% once newer device models were in use |
| Meta-analysis of 28 studies, 13,078 patients (Mah et al., 2025) | 0.22%, on evidence graded low-certainty |
| The manufacturer (Allergan CoolSculpting FAQ) | About 1 in 3,000 treatments (0.033%) |
Who is at higher risk. ASPS reports that men develop PAH three times as often as women and that it is seen more in people of Hispanic or Latino descent (ASPS on cryolipolysis). The 2025 meta-analysis did not find a statistically significant difference by sex (Mah et al., 2025).
What happens if it occurs. The 2014 authors saw nothing to suggest PAH goes away by itself (Jalian et al., 2014). A 2018 case series followed 11 patients. Seven of them had surgery: six had liposuction and one had liposuction combined with abdominoplasty. The surgeons waited until the tissue softened, typically 6 to 9 months after the cryolipolysis, and three of the liposuction patients later needed a second procedure (Kelly et al., 2018).
Have the provider walk you through PAH before you consent to CoolSculpting.
Chin
Kybella for fat under the chin
Kybella (deoxycholic acid) is FDA-approved to improve the look of moderate to severe fullness from submental fat in adults. One session can involve as many as 50 small injections, and a patient may have as many as 6 sessions, each at least 1 month after the last. The two placebo-controlled trials gave Kybella to 513 patients and placebo to 506 (Kybella label).
The approval covers the chin area and nothing else. The label says use on fat outside the submental region is neither approved nor recommended, and lists serious reactions reported after such use: muscle or nerve injury and, when injected close to the eyes, blurred or reduced vision (Kybella label). A provider who proposes Kybella for another body area should explain why.
What the trials and label record:
- Uneven smile. Marginal mandibular nerve injury occurred in 4% of Kybella patients. Every case cleared up without treatment, after anywhere from 1 to 298 days, with a median of 44 days.
- Swallowing. Trouble swallowing occurred in 2% of patients.
- Numbness. Swelling, bruising, pain and numbness are common, and 42% of patients had numbness lasting over 30 days.
- Injection-site problems. The label also lists ulceration, necrosis, nodules, masses and infection where the drug was injected (Kybella label).
This site lists no Dallas price range for Kybella or for any of the fat devices above.
Muscle
Muscle: Emsculpt Neo and HIFEM
What the clearance covers. Emsculpt Neo is cleared for non-invasive lipolysis and a reduced abdominal circumference (FDA clearance K192224).
The clearance study. Forty-two people with a BMI of 35 or lower were enrolled. At three months, mean abdominal circumference was down 3.2 cm and ultrasound showed mean fat down 29.8%. The study had one arm and no controls (FDA clearance K192224). The BMI cap of 35 was only an enrollment rule; it is not written into the cleared indication.
The original Emsculpt. This earlier device stimulates muscle with no radiofrequency. An MRI study measured 22 patients two months after four sessions and found fat thickness down 18.6%, rectus abdominis thickness up 15.4% and abdominal muscle separation reduced by 10.4% (Kinney and Lozanova, 2019).
The wider evidence. A 2023 systematic review collected 15 clinical studies of Emsculpt and Emsculpt Neo; only two covered Emsculpt Neo. Pooled abdominal results showed mean fat thickness down 5.5 mm, muscle thickness up 2 mm and muscle separation improved by 3.0 mm. Two of the studies found marginal or no benefit, and no study recorded any change in weight (Kohan et al., 2023).
Skin
Tightening skin, and where devices stop
Energy devices tighten skin by heating it so the tissue contracts. Removing skin is different; it takes an operation, a tummy tuck being one example (ASPS on tummy tuck).
Morpheus8 (fractional RF microneedling)
An array of pins carries radiofrequency energy into the skin. The clearance covers dermatologic procedures needing hemostasis or the coagulation or contraction of soft tissue (FDA clearance K231790). An FDA safety communication about RF microneedling, dated October 15, 2025, says the agency is aware of reported nerve damage, disfigurement, fat loss, scarring and burns, some of which needed medical care or surgical repair (FDA on RF microneedling).
BodyTite (RF-assisted liposuction)
A search of the FDA 510(k) database under the name BodyTite returns no entry. Ask the provider for the 510(k) number that covers the device and what that clearance allows.
Renuvion / J-Plasma (helium plasma and RF)
The FDA warned in March 2022 against using Renuvion/J-Plasma for skin contraction and dermal resurfacing; at that point it had not been found safe or effective for any aesthetic skin procedure. After narrower clearances, the FDA updated the communication on May 10, 2023 (FDA on Renuvion/J-Plasma). Two handpieces now have clearances:
- Renuvion Dermal Handpiece: moderate to severe wrinkles, for Fitzpatrick skin types I to III (FDA on Renuvion/J-Plasma).
- Renuvion APR Handpiece: lax skin of the neck and submental region, and coagulation of subcutaneous tissue after liposuction for body contouring (FDA clearance K230272).
If your area is not on either list, ask why the provider is proposing it.
Cellulite
Cellulite
Avéli. A handheld tool goes through a small incision and cuts the fibrous bands (septa) that tether cellulite dimples (FDA clearance K221336). The FDA cleared it for long-term reduction in the appearance of cellulite on adult women’s buttocks and thighs, with data supporting benefit out to one year. Seventy-four women had one treatment each in the pivotal study. It reached its primary endpoint, a mean gain of more than 1 point on the Cellulite Severity Scale at 3 months, and the improvement held at 6 and 12 months. Bruising was the most frequent side effect (86.8%), followed by tenderness (51.5%), pain (38.2%), firmness (36.8%) and numbness (17.6%) (FDA clearance K221336).
truSculpt massage device. Cleared for a temporary reduction in the appearance of cellulite (FDA clearance K223110).
Qwo. On December 6, 2022, Endo said it would end production and sales of this injectable cellulite drug, pointing to concerns about bruising and the possibility of long-lasting skin discoloration (Endo SEC filing).
Liposuction. ASPS says it is not an effective cellulite treatment (ASPS on liposuction).
GLP-1
After GLP-1 weight loss
Losing weight on a GLP-1 drug removes lean mass as well as fat. In a SURMOUNT-1 substudy, 160 participants were followed for 72 weeks. On tirzepatide they lost 21.3% of body weight, 33.9% of fat mass and 10.9% of lean mass, so lean mass was about 25% of the total loss, the same share as on placebo (Look et al., 2025).
Once the weight is off, what to do depends mostly on the skin that remains:
- Cryolipolysis may be a poor fit when tone is poor or skin is loose (ASPS on cryolipolysis).
- Energy devices such as Morpheus8 can coagulate or contract soft tissue under their clearance (FDA clearance K231790), but they do not take skin away.
- Liposuction reshapes skin less well when weight loss has left it thin and soft, according to ASPS, and further surgery to remove the excess may follow (ASPS on liposuction).
- A tummy tuck removes surplus skin and fat; ASPS advises waiting if more substantial weight loss is planned (ASPS on tummy tuck).
If you are on a GLP-1 drug, tell the provider before any procedure and ask whether it changes the plan.
Surgery
Surgical options
Your consultation exam answers this, and three things count most: skin laxity, muscle separation and body weight.
- Liposuction surgically removes surplus fat from chosen areas. ASPS says good candidates are at a normal weight with some extra fat, and that results are better when skin is firm and elastic. It is not a treatment for obesity and does not stand in for diet and exercise (ASPS on liposuction).
- Abdominoplasty (tummy tuck) removes surplus skin and fat and usually repairs separated or weakened abdominal muscles. Stretch marks remain, and it is no substitute for weight loss. ASPS advises postponing it ahead of a planned pregnancy or substantial weight loss (ASPS on tummy tuck).
- Non-surgical fat reduction works on one fat bulge at a time, and ASPS says none of these methods replaces liposuction (ASPS on nonsurgical fat reduction).
If you are weighing surgery, ask the surgeon about board certification and look up their Texas license on the Texas Medical Board’s Look Up a License page.
What to Expect
The visit, the treatment and the weeks after
At the consultation. Expect questions about GLP-1 drugs, pregnancy and breastfeeding, cold-related conditions (for cryolipolysis), and swallowing or bleeding problems (for Kybella) (ASPS on cryolipolysis) (Kybella label). The provider should also establish whether your concern is fat, muscle, loose skin or cellulite.
After treatment. The timeline uses the published figures for each treatment. Your own plan and follow-up dates come from your provider.
-
Treatment day
Avéli is a single treatment through a small incision (FDA clearance K221336). Kybella involves as many as 50 small injections under the chin (Kybella label). A cryolipolysis treatment takes about an hour, and ASPS lists no activity restrictions after it (ASPS on cryolipolysis).
-
The first few weeks
After Avéli, 73.5% of study patients were doing normal activities inside a week, and bruising lasted a median of 27 days (FDA clearance K221336). Kybella commonly brings swelling, bruising, pain and numbness (Kybella label). Cryolipolysis can leave a soreness some people compare to the day after a workout (ASPS on cryolipolysis).
-
Months 1 to 3
The Avéli study took its primary measurement at 3 months (FDA clearance K221336). The Emsculpt Neo clearance study measured patients 1, 2 and 3 months after their last session (FDA clearance K192224). Further Kybella sessions come at least 1 month apart (Kybella label).
-
Months 4 to 6
Treated fat cells from cryolipolysis are cleared over this four to six month window (ASPS on cryolipolysis). An area that becomes larger and firmer instead of shrinking should be checked for paradoxical adipose hyperplasia (Jalian et al., 2014).
-
12 months
Avéli's pivotal study showed the result lasting out to one year (FDA clearance K221336).
Cost
What it costs in Dallas
This site does not publish a Dallas price range for body contouring. The total depends on the device, the number of areas and the number of sessions. Get the full figure in writing, and have it spell out required packages, follow-up visits, and the policy for touch-ups and complications.
Combinations
Pairing treatments
Fat, muscle and skin respond to different methods, so plans often combine them. For each item in a combined plan, ask what it is for, where it falls in the sequence, and what its FDA clearance covers. Related pages: RF microneedling, dermal fillers, and wellness and HRT.
Texas Law
Choosing a provider around Dallas
The Texas rules. Delegated non-surgical medical cosmetic procedures are governed by 22 TAC Chapter 169, Subchapter E, as amended by Texas Medical Board rule changes in effect from January 9, 2025 (Texas Medical Board). What they require:
- A relationship first. Before a delegated procedure, a physician must establish a practitioner-patient relationship with you, or a PA or APRN may do it under the physician’s delegation. Clinics often call this the good faith exam (22 TAC §169.26).
- Someone available. During the procedure, a physician, PA or APRN has to be on site or immediately available for emergency consultation if something goes wrong (22 TAC §169.26).
- Paperwork. Delegation needs a written order naming the delegating physician and covering patient screening criteria, a description of appropriate care, and how complications and emergencies are handled (22 TAC §169.27). The person performing the procedure signs and dates a written protocol (22 TAC §169.26).
- A posted license. The delegating physician’s name and Texas license number must be on display in every public area and treatment room (22 TAC §169.28).
What to ask:
- Which physician delegates, and which person will actually treat me?
- Is the device an authentic, current model from the manufacturer? One multicenter study saw PAH rates fall by over 75% after newer CoolSculpting models arrived (Nikolis and Enright, 2021).
- Does the device’s FDA clearance cover my area?
- For CoolSculpting: what do you tell patients about PAH, and what is your plan if it happens?
- What reduction should I expect, and how will you measure it? ASPS gives about 20 percent on average for cryolipolysis (ASPS on cryolipolysis).
- What is the full price, and how do you handle complications and revisions?
